I think of Spider-man and how he is always willing to give of himself. Most of the time, it hurts the people who love Peter Parker, causing him to feel guilt for causing them pain...doubling his burden. He doesn't get what he wants, plus he's hurting those he loves...all to do the right thing.
For the second part of the question, I can't think of any specifics but it seems like there have been times in a movies in which a "right hand man" sacrifices himself for the main bad guy. Usually this is caused by main bad guy. While the Lieutenant was willing to give of themselves, the main bad guy takes advantage of this and exploits it to attempt escape.
